Objective: 

• To ensure patients undergoing treatment with Coumadin/Warfarin have INR levels within therapeutic range

 

Program Activities:

• Nurse provides phlebotomy (blood work) services

• RN closely monitors INR levels and manages medication for patients undergoing therapeutic treatment with Coumadin/Warfarin
