About Us

A Family Health Team is a group of healthcare providers working together to provide primary care to patients. 

Temagami Family Health Teams include physicians, nurse practitioners, registered nurses, registered practical nurses, pharmacists, dietitians, social workers, psychotherapists, respiratory therapists, and occupational therapists among others.

In addition to traditional healthcare services, family health teams focus on health promotion, illness prevention and chronic disease management.

Our team provides primary care services at three clinic sites located in Temagami, Latchford and Cobalt. Our team currently serves over 4,500 patients.
