Full Time Permanent PharmacistPosition Summary                                                                                               
As a key member of the four Family Health Teams (Temagami, Great Northern, Englehart and Haileybury), and an integral part of an interdisciplinary health care team, you will deliver pharmaceutical care in accordance with FHTs policies and professional practice standards. You will perform routine pharmaceutical duties, and ensure that the pharmaceutical needs of our patients are met.
 
The successful candidate may identify any of the four sites as their home base and will work under the direction of the Steering Committee and according to the annual work plan that will be developed and implemented.  The position will require the candidate to work remotely with travel to the various sites each week.
 

Roles and Responsibilities
  • Perform patient medication reconciliation
  • Consult with interdisciplinary team members and physicians
  • Educate targeted patients
  • Interprofessional education
  • Individualized care planning
  • Develop policies & procedures to improve medication use
  • Operate pharmaceutical clinics
  • Improve transitions of care, including increased communication between hospital & community pharmacists

Skills, Knowledge Requirements
  • Graduate from a recognized pharmacy education program and current registration with the Ontario College of Pharmacists
  • 3 years of related experience in a community, hospital or primary care setting
  • Exceptional understanding of current, evidence based and best practice pharmacy guidelines
  • Excellent oral / written skills
  • Strong clinical skills
  • Autonomous and able to function with minimal supervision
  • Ability to work effectively with all members of the health care team
  • Ability to prioritize, manage time effectively and be flexible in a very active work environment
   Computer Skills
  • Basic computer skills
  • Experience with electronic medical records (EMR)
  • Familiar with pharmacy programs and drug information software
  • Ability to perform Internet based literature searches on electronic databases
   Language Skills
  • English required; French not required but would be an asset.
