In Family Health Teams, family doctors work with other health care professionals such as nurse practitioners, nurses, social workers, dietitians, occupational therapists and pharmacists to see patients and to keep them healthy. Our Family Health Teams focus on preventing illness and promoting health.

The Great Northern and Temagami Family Health Teams (FHT) are seeking a full-time occupational therapist to work across both teams equally as a shared resource.  Our Occupational Therapist will offer assessments for patients with chronic conditions or disability around activities of daily living with a goal to assist them to optimize function and quality of life.
OT services will include, but are not limited to:
  • Assessment of physical, cognitive, perceptual and environmental issues that may be contributing to a person’s safety or difficulty managing daily activities
  • Development of person centred goals
  • Recommendation of practical interventions to optimize safety and independence within their environments
  • Neuro-cognitive and functional assessments related to dementia and memory deficit
  • Chronic Pain Management
 
Interventions may include:
  • Consultation regarding assistive devices, adaptations or mobility
  • Education related to falls prevention and home safety
  • Strategies to help self-manage chronic conditions and optimize health and well-being         
Related Considerations/Duties
  • Models the values and philosophy of the FHTs
  • Provides mentorship and additional skills training for co-workers and students where identified
  • Maintains current knowledge of accepted operating policies and procedures
  • Collects data as required for statistical/reporting purposes
  • Provides timely and informative reports as directed and requested by the Executive Director and/or Physician Lead
  • Participates in team building, program development, and improvement activities
  • Participates in staff meetings
  • Participates in established performance evaluation process and professional development goals
  • Develops, updates and maintains education resources

Skills/Knowledge Requirements:
  • Outstanding problem-solving and conflict resolution skills
  • Familiar with home assessments
  • Familiar with cognitive testing and complex geriatric assessments
  • Ability to take a collaborative patient history
  • Has worked in a geriatric setting and understands related issues
  • Understanding of community resources
  • Knowledge and understanding of Continuous Quality Improvement
  • Strong problem-solving, decision-making and time management skills
  • Superior interpersonal skills
  • Exhibits an ability to be open and non-judgmental
  • Ability to work within a team or independently
  • Knowledge and proficiency in current, evidenced-based methods/ practices with geriatric focus
  • Prior experience and knowledge of electronic medical records
  • Willingness to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences
  • Work in compliance with Health and Safety laws adhering to safe work practices and procedures established by the Great Northern and Temagami FHTs
  • Other duties as required
Computer Skills
  • Experience with scheduling and electronic medical record keeping using a clinical management system (Telus Health PS Suite) is an asset
  • Familiarity with Microsoft Office
Education/Specific Job Requirements:
  • Completion of appropriate academic preparation (e.g. MSc.OT)
  • Additional training or experience in chronic disease management and geriatrics is preferred
  • Registration with the College of Occupational Therapists of Ontario and evidence of liability insurance
  • Flexibility to work evenings, if required
  • Police Record/Vulnerable Sector Check
Priority will be given to applicants with experience in cognitive testing, geriatric and home assessments; in developing community linkages and relationships; and in program development, implementation, monitoring and evaluation with a focus on adults or geriatrics.
